IN THE ABSENSE OF A SUPERVISOR, NOTIFY THE HEALTH AND SAFETY REPRESENTATIVE FOR THE AREA OR A MANAGEMENT REPRESENTATIVE.
IF NONE OF THESE ARE AVAILABLE, TIE AN “OUT OF ORDER” TAG IN A PROMINEMT POSITION ON THE EQUIPMENT. MAKE SURE YOUR NAME AND THE DATE ON THE TAG. ADVISE A SUPERVISOR AS SOON AS POSSIBLE.
YOU MUST NOT USE ANY EQUIPMENT TAGGED AS “OUT OF ORDER” OR REMOVE AN “OUT OF ORDER” TAG.
AN “OUT OF ORDER” TAG MAY ONLY BE REMOVE BY
1. A QUALIFIED REPAIRER, AFTER REPAIRS HAVE BEEN COMPLETED,
2. A SUPERVISOR, FOLLOWING CONSULTATION WITH A HEALTH AND SAFETY REPRESENTATIVE.
